Please fill out and sign the appropriate statement to either give or to decline permission to use pictures and video of you and/or your minor during SLAM Student Ministry promos, at other events, online, television broadcasting, etc. With regard to the use of photos and videos for publicity, it is our policy that persons in photos not be identified by name. Please note that these photos and videos will be used now and in the future.
Grant Permission to use you or your minor's pictures & videos
I GRANT permission to SLAM Student Ministry & Metro North Church to publish pictures & videos of myself and/or the minor named above on the SLAM Student Ministry & Metro North Church websites, in promotional videos, press releases, or any other form of public publicity.
If signing for a minor, I further state that I have the right to give this permission to the minor named above as I am the minor’s parent or legal guardian. I understand that if I give notice to SLAM Student Ministry or Metro North Church that I object to any particular picture or video on the website, it will be removed as soon as possible.
I DECLINE to grant permission for SLAM Student Ministry & Metro North Church to publish pictures & videos of myself and/or the minor named above on the SLAM Student Ministry or Metro North Church websites, in promotion videos, press releases, or any other form of public publicity.
